Your Prime Med experience begins when you visit any of our three convenient locations in Orange Park, Jacksonville's Southside, or in Historic St. Augustine. Upon arrival, you will immediately be welcomed by our competent and friendly medical staff.

To save time, please complete the forms and bring them with you on your scheduled visit to Prime Med along with your photo ID and any of your current Healthcare Insurance Cards.
Don't forget to sign the Patient Portal Consent Form. Once your login I.D. has been activated by our office, you can login and take advantage of all of our website's online patient features! Through our portal, you can access certain part of your chart, as well as update your demographics at any time.
